In der Medizinethik sind der Respekt vor der Patientenselbstbestimmung, das Nichtschadensgebot, das Handeln zum Wohl des Kranken und das Gerechtigkeitsgebot praxisrelevante Prinzipien. Anhand des Beispiels der Frühintervention und Akutbehandlung schizophrener Störungen wird aufgezeigt, dass es in der psychiatrischen Praxis zu einer Kollision dieser Prinzipien kommen kann. Der frühe Krankheitsbeginn und der häufig chronische Verlauf schizophrener Störungen führen zu großem Leid der Betroffenen und ihrer Angehörigen sowie zur ökonomischen Belastung der Solidargemeinschaft. Die negativen Folgen einer verzögerten Intervention stehen den Risiken der (...) Stigmatisierung sowie den möglichen Nebenwirkungen durch die psychotherapeutische oder psychopharmakologische Behandlung gegenüber. Es wird für eine an der konkreten Situation orientierte, lebensnahe und dynamische Ethik plädiert, die vom konkreten Fall aus diskutiert und verschiedene Perspektiven berücksichtigt – mit dem Ziel von einer Arztethik und Patientenethik zu einer Verantwortungspartnerschaft zu gelangen. Die demografische Entwicklung und der medizinische Fortschritt werden die Problematik der Ressourcenknappheit im Gesundheitswesen in Zukunft weiter verschärfen. Auch Deutschland steht eine Prioritätensetzung im Gesundheitswesen bevor. Diese sollte in möglichst transparenter Weise nach klaren Kriterien erfolgen. Ein nur selten in diesem Kontext besprochenes Kriterium der Verteilung von Mitteln in der Gesundheitsversorgung ist die marginale Wirksamkeit medizinischer Leistungen. Im vorliegenden Beitrag wird dieses Kriterium vorgestellt und auf seine Fairness hin untersucht. Nach der kritischen Diskussion einiger Argumente gegen den offenen Einsatz von (...) Rationierungskriterien und einer Definition von marginaler Wirksamkeit in zwei unterschiedlichen Dimensionen werden verschiedene Vorzüge und eventuelle Nachteile des Kriteriums beleuchtet. (shrink)

Successful scientists pick out one philosopher as having articulated the rationality of what they do as scientists. He is Sir Karl Popper FRS. But Popper's ideas play no part in contemporary philosophy. As Popper has said "Here I am being showered with honours as no professional philosopher before me; yet three generations of professional philosophers know nothing about my work" (Bartley, 1982). How did this situation arise? I suggest, because philosophers use a false analogy to model the nature of authority (...) held by ideas. Commonsense says we are isolated. After all, our bodies are physically separate. But Seneca’s colamus humanitatem, and John Donne’s observation that “no man is an island” suggests we are neither entirely isolated nor separate. A recent discovery in neuroscience—that of mirror neurons—argues that the brain and the mind is neither built nor functions remote from what happens in other individuals. What are mirror neurons? They are brain cells that process both what happens to or is done by an individual, and, (...) as it were, its perceived “refl ection,” when that same thing happens or is done by another individual. Thus, mirror neurons are both activated when an individual does a particular action, and when that individual perceives that same action done by another. The discovery of mirror neurons suggests we need to radically revise our notions of human nature since they offer a means by which we may not be so separated as we think. Humans unlike other apes are adapted to mirror interact nonverbally when together. Notably, our faces have been evolved to display agile and nimble movements. While this is usually explained as enabling nonverbal communication, a better description would be nonverbal commune based upon mirror neurons. I argue we cherish humanity, colamus humanitatem, because mirror neurons and our adapted mirror interpersonal interface blur the physical boundaries that separate us. (shrink)

Science is about evaluation, persuasion and logic. In scientific debate, scientists collectively evaluate theories by persuading each other in regard to epistemological qualities such as deduction and fact. There is, however, a flaw intrinsic to evaluation-by-persuasion: an individual can attempt and even succeed in persuading others by asserting that their reasoning is logical when it is not. This is a problem since, from an epistemological perspective, it is not always transparent nor obvious when a persuasive assertion is actually deductively warranted. (...) Empirical research upon reasoning, indeed, supports the notion that assertions are often strongly persuasive for reasons other than their logic. The unreliability of the link between persuasion and logic raises an important methodological issue: how do scientists debate in a manner such that claimed but false “logical” arguments are ignored, and only warranted arguments get to determine theory preference? This need for soundness in debate is a particularly important epistemological concern in cases where the deductive qualities of persuasive argument are not overt, and so cannot be directly checked --such as when they are founded upon Bayesianism probabilistic coherence. The argument presented here is that scientists make the qualities of probabilistic and nonprobabilistic inference sound (and so warranted) through how they organize their debate. Scientists, I argue, abide by “epistemological quality control practices” that limit the persuasive power of unsound arguments upon theory evaluation. Examples of such debate quality control practices are publicness, clarity, openness to criticism, and the collective promotion of attempts at theory conjecture and refutation. Methodologically, these quality control practices are extralogical since they do not directly provide scientific inferences with additional logical warrantedness. They function instead in science to generate an epistemological evaluative environment in which persuasiveness is due, and only due, to logic (i.e. sound). Their methodological role is therefore to make what is warranted de papyri (in our principles of rationality – epistemologically competence) also what is persuasive in evaluation and debate de cognitio (in our exercise of such principles – epistemologically performance). Several limitations exist upon the soundness of Bayesian inferential coherence – surreptitious revision, logical omniscience, uncertain evidence, old evidence, and new hypotheses. Bayesianism, as a result, can only exist if it is pursued in a debate that is regulated by quality control practices over its inferences (for instance, practices that ensure there is autonomy of inference, diligence of inference, probity of evidence, auditability of inference and assiduousness of conjecture). In this context, one can reinterpret Popper’s concern with criticism, openness, refutation and conjecture, as deriving not (as he thought) directly from the needs of logic, but, indirectly, from the need of scientists to create epistemological soundness. Basham and Dentith argue that the danger of condemning both conspiracy theorists and their conspiracy theories in a democracy has grave consequences. They argue that we should encourage research into public concerns about influential institutions, especially in cases where a conspiracy has been alleged. Rather than dismiss conspiracy theorising, we should, encourage the politically crucial, historically proven gift of watchfulness in the citizen, and its sometimes necessary, proper and correct expression, conspiracy theory.

The placebo drug reactions from controlled trials were studied for the first time systematically for efficacy and the safety in drug data pooled from randomized, placebo-controlled, multicentre studies. Results: The efficacy of placebo on clinical symptoms and outcome varied between the therapeutic indications. However, no placebo effects on laboratory values, as e.g. blood glucose or Hb1c in diabetics, were noted. The frequency and type of placebo-induced adverse reactions also varied between indication groups. The placebo side effect profile was largely similar (...) to the side effect profile of the active treatment. The mechanisms of placebo effects are manyfold and varied (e.g. endorphin release, conditioning), much lacks explanation. Conclusion: Since the prescription of non-evidence based medicines (= pseudoplacebos) may clearly also result in serious adverse effects, such practice may not only be non-beneficial but may even be harmful. In clinical research, the judicious use of placebo remains essential to establish the efficacy and safety, safeguarding that patients receiving placebo will not be subject to harm and are fully informed. Purpose: The purpose of the study was to assess the socio-economic challenges faced by CoG-Kenya in poverty alleviation in Emuhaya District, Western Kenya.Methodology: This study was a cross-sectional research that sought to give an examining and descriptive scrutiny of the CoG-K’s activities in Emuhaya District of Western Kenya. This study sampled a total of 312 respondents (1 Bishop, 1 General Secretary, 1 General Assembly Trustee, 1 General Assembly Treasurer, 16 Directors, 282 Pastors, and 10 Elders) through purposive sampling method. Oral (...) interviews, questionnaire methods and focus group discussion were used to collect data from the respondents. Observation method was used to validate the information obtained through oral interviews and questionnaires. Secondary data was sourced from published and unpublished literature. Data was analyzed using the theological method of induction (TMI).Results: The study concludes that as much as the CoG-K has made concerted attempts towards development in Emuhaya District by influencing the peoples thinking on legitimacy of wealth and education, and on the moral value of saving and investing. However some of its programs and ceremonial functions like ordination, baptism, marriage and weddings tend to retard these efforts of alleviating poverty. Although such programs and celebrations serve the poor at the grassroots level, they delay change in behavioural and institutional transformation capable of sustaining economic growth.Unique contribution to theory, practice and policy: There is a need to change cultural practises that tend to retard development among the adherents of the CoG-K in Emuhaya District. The study posits that cultural practices surrounding marriage and death that are repugnant and detrimental to change should be discarded if development is to take place. (shrink)

A review based study of music (geet, sangeet) was done in these classics to explore whether these classics includes any form of music as therapy or not. Based on review of these classics it was (...) found that music in form of geet (vocal or instrument) has been given in contexts like vajikara(aphrodisiac), vata/pitta prakriti (basic consititutional make up), gandharva sattva, madyapana (intake of alcohol), therapy for rajyakshma (immune deficiency diseases), sanyasa (syncope/coma), jivadan (haemetemesis) etc. various swara have been correlated with their pacifying power of three sharirikadoshas viz. vata, pitta and kapha. Also a relationship between various raga of indian classical music have been said to be based on time of its singing which may be used as a therapy to pacify the dearrangement of prevalent doshas. several articles have shown music as therapy in sleep disorder, psychiatric disorders, schizophrenia, cardiovascular, cancer etc. with this paper a perspective of Ayurveda have been explored in music therapeutics. This daivyapashraya includes mantra, ausadhi, mani, mangal, bali, upahaar, homa, niyama,prayashchita, upawaas, swastyayana, pranipata, gamana etc. These type of methods are widely described in karma kanda of mimamsa darshana. This treatment method has been used in curing of diseases like unmada,graharoga. Methods of yagya has been described as well as used in putreshti and punsawansamskara described in charakasamhita. In his very interesting book The Grounds of Moral Judgement, Dr. G. R. Grice tries to reconstitute contract theory so as to give an account of morality such that moral requirements can be explained in terms of what he calls the contract ground. He wants to go on and argue from this that it is irrational to be immoral, but my concern lies immediately with the contract ground. I think that faults can be found in the setting up of the (...) contract ground, which is the core of Grice's theory. (shrink)

Purpose: The purpose of this study was to investigate the extent to which some selected parental factors influence career development among form four students in Isinya Sub-County.Methodology: The study adopted a descriptive survey. The population of the study was all the 572 form four public school students in Isinya Sub-County. The study focused on all the nine public secondary schools from Isinya Sub-County to carry out the survey while proportional random sampling technique was used to sample the students. The researcher (...) used a sample of 114 students from the target population. Questionnaires and interview guides were used as the instruments for data collection. Statistical Package for Social Sciences (SPSS) software was used to analyse the data. Pearson correlation was test relationships between variables and their significanceResults: Results indicate that the father’s education level was significant and negatively correlated with the students career choice (r = -.067, p = 0.0445) at 0.05 significance level. Parent child relationship is of paramount importance as it reinforces positive affirmations on the students that help them excel in life. Thus, most parents / guardians of told the students that they are proud of them. Majority of the parents encouraged students to ask questions about different careers.Unique contribution to theory, practice and policy: The study recommends that parent child relationship should be embraced as it is of paramount importance in reinforcing positive affirmations on the students that help them excel in life. The third international conference on classical influences took place in Cambridge in 1977 under the title 'Classical Influences in Western Education, Philosophy and Social Theory'. Dr Bolgar has here collected and edited the proceedings and produced a volume which attempts to relate the progress of classical studies to the general history of ideas from the mid-seventeenth to the mid-nineteenth century. The book should be of interest to specialists in classical studies, to students of the literature of the period, and to (...) students of eighteenth-century French, Italian and American history. If one wishes to accept that some painful animal experimentation can be justified on grounds that benefit is conferred, one is faced with a difficult moral dilemma argues the first author, a philosopher. Either one needs to be able to say why human lives of any quality however low should be inviolable from painful experimentation when animal lives are not; or one should accept that sufficient benefit can justify certain painful experiments on human beings of sufficiently low quality of life. (...) Alternatively, one can reject the original premise and accept antivivisectionism. In this issue of the journal “Lee Elder”,1 a pseudonymous dissident Jehovah's Witness , previously an Elder of that faith and still a JW, joins the indefatigable Dr Muramoto2–5 in arguing that even by their own religious beliefs based on biblical scriptures JWs are not required to refuse potentially life-saving blood transfusions. Just as the “official” JW hierarchy has accepted that biblical scriptures do not forbid the transfusion or injection of blood fractions so too JW theology logically can and should (...) permit the transfusion of whole blood when this is medically required.Few doctors would argue that they should override the adequately autonomous decisions of Jehovah's Witnesses to refuse blood transfusions even if they are likely to die as a result of such transfusions. However, there is a case to be made for doctors asking such patients to reflect on their potentially fatal refusal of blood and for drawing to these patients' attention the reasoning of members of their own faith that justifies acceptance of potentially life-saving blood transfusions. What is that case? Simply that doctors' primary professional duty to try to benefit the health of their patients entails trying to save their patients' lives when and if doing so will benefit their patients' health. Of course this is not an absolute duty overriding all other duties; in particular if patients who are adequately autonomous to do so refuse such life-prolonging treatment doctors must generally accept such refusal, however sadly. This editorial endorses that view in the case of adequately autonomous legally competent JWs.
